Below is a sampling of some medical journals. A PubMed search is probably the easiest way to dig for journal articles of interest. There you will find abstracts, with links to free full text articles and, tragically, some articles locked away in pay per view dungeons. Use your favourite repository or friends to freely access anything in full text.

Digital Journal of Ophthalmology Free full text online peer reviewed clinical journal with good tutorials and case presentations from Harvard.

Canadian Journal of Ophthalmology Used to be free full text, now part of Elsevier.

eBMJ, British Medical Journal. Free full text after one year. One of the big peer reviewed pay per view general medical journals groups.

General Basic Science

The Royal Society is a Fellowship of many of the world's most eminent scientists and is the oldest scientific academy in continuous existence. Their free content policy is here.

Journal of Neuroscience Free after one year.

Brain Free after two years.

JAMA Neurology Used to be Archives of Neurology. Free after one year.

http://neuroscion.com Elsevier-Reed's compendium of neuroscience journals. Some free trial periods from time to time.
The deal is researchers, who are often financed in part or fully with public funds, often building upon unrestricted prior published work and supported by society in general, write a paper. They give the articles to the publisher for free so that the private publisher, Elsevier, can get richer charging researchers and others exorbitant rent to read the articles, placing restrictions on the prior unrestricted reference data used to build a paper and maintain its restrictive, stifling system. Sustainable open access publishing models exist and should be encouraged. Academic journal publishing reform is required now in the era of the disruptive, knowledge liberating tool, the internet.
